    Questionnaires for downloading

    These questionnaires may be downloaded and used free of charge. Permission is not needed.  Some of the questionnaires have been translated into languages other than English and I am grateful for those who have done this.  If the questionnaire is needed in another language feel free to translate using the conventional translation-back-translation methods. I have made the questionnaires open access so that students and researchers have easy access.

    Details of my publications relating to these questionnaires is available from my list of publications.  However, I would recommend you do a literature search using the questionnaire name as keywords if you want further information as some of these questionnaires have been used extensively with publications by other authors.
